Dear Insurance Trust for Delta Retirees Member,

When paying for CarelonRx home delivery prescriptions using your Optum Bank Mastercard® Payment Card, you may be asked by Optum to validate the expense is healthcare related.  We apologize for this inconvenience.  We are working to adjust our system so that later this year you will not need to provide documentation for CarelonRx Pharmacy expenses.

Please refer to the fact sheet below for information on how to use your card and submit documentation to verify your payment.

If you need any help with your home delivery prescriptions, please call our dedicated support line at 833-476-2694 (TTY: 711) from 7 am – 7 pm Eastern. We look forward to continuing to serve you.

Delta’s Optum Bank Mastercard® Payment Card Facts

Quick, Convenient Access to Your Funds

Your plan lets you use your Mastercard Payment Card like a debit card for eligible expenses under your plan.

• Immediate access to funds. Use your payment card, versus submitting your claims online or by fax/mail, so you don’t have to worry about reimbursements later – the payment is automatically taken from your Optum Bank account and delivered to the merchant.

• Convenience. Use your payment card like a debit card. Just remember to keep your receipts, because certain transactions have to be validated, or substantiated, per IRS guidelines.

Submitting Receipts Is Easier Than Ever

Although you do not have to file a claim for reimbursement with your payment card, currently if you purchase a prescription through CarelonRx, you may be required to submit documentation verifying your payment. In that case, Optum Bank will send you a letter to request more information and to remind you of the need to submit the documentation in a timely manner. To submit documentation, use one of the methods below:

• Scan your Explanation of Benefits (EOB) or receipt and upload it online at optumbank.com. Simply log into your account and select the specific transaction, click on “Upload receipt” and you’re done.

• Email, fax or mail – be sure to include a copy of the Receipt from CarelonRx.

Sincerely,

Your Anthem and CarelonRx teams
